The last line of the Tur talks about קמטים that women attached to their leggings and sleeves, which is forbidden to do on YT because of tikkun kli. I have no idea what קמטים are. Anyone know?
That said, my understanding of the history of fashion is that collared shirts, with the collar attached to the shirt, is a somewhat modern invention from the first half of the 20th century CE and that prior to that collars and cuffs were attached to the shirt. See here

Would it be forbidden to attach a collar or cuff to a shirt, lets say with buttons or other fasteners, on YT because of being metakein a kli?

I don't know. I assume even temporarily since I didn't see anything to indicate otherwise, like if you attach it with intent to remove it within 24 hours it's ok.

Usually it means wrinkles, here I believe it means something like pleats.
